How can I create QTVR with Panorama Tools in Windows?
If you want convert to the new cubic .mov format you can use updated some
days ago PanoCube utility. It accept direct equirectangular input now
and works with TIF, BMP and JPG files.

'Old-style' cylindrical QTVR panoramas can be created with the
VRMakePano tool from Apple:

http://developer.apple.com/samplecode/Sample_Code/QuickTime/QuickTime_VR/VRMakePano.htm

In PTStitcher/PTGui select Cylindrical and QTVR as the output format. The
resulting .bmp can be converted to .mov using the above tool.
